Supplements are widely assumed to be too mild to interfere with medication. The mechanisms that produce drug interactions do not distinguish between a prescribed molecule and one bought over the counter.

Most interactions run through one enzyme family

The liver clears a large share of medicines using a family of enzymes that chemically modify compounds so the body can excrete them.

Any substance that increases the activity of those enzymes speeds up clearance of everything else they handle, lowering the effective level of a medicine.

Any substance that inhibits them does the reverse, allowing a medicine to accumulate. Both directions are clinically significant, and neither produces an obvious sensation.

Botanical extracts are the most common source

Plants produce complex mixtures of compounds, several of which may be active on these enzymes even when the extract is standardised for something else.

Because the composition varies with the plant material and the extraction method, the strength of the interaction can differ between products carrying the same name.

This variability is why interaction warnings for botanicals tend to be broad, and why a previously uneventful product can behave differently after a change of supplier.

Absorption interference works differently

Some interactions occur in the gut rather than the liver. Minerals such as calcium, magnesium, iron and zinc can bind certain medicines and prevent them being absorbed.

Here the effect depends on timing. Separating the two by several hours often avoids the problem entirely, which is not true of enzyme-level interactions.

Fibre supplements can have a similar effect by slowing transit and binding compounds in the intestinal contents.

Additive effects need no shared pathway

A supplement and a medicine can act on the same physiological system independently and still combine. Anything affecting clotting, blood pressure or blood glucose falls into this group.

The individual effect of the supplement may be modest and still matter, because the medicine has already moved the measurement towards its intended target.

This category is easy to miss, since neither product would raise concern taken alone.

Why disclosure is the practical step

Patients frequently omit supplements when asked what they take, on the reasonable-sounding basis that they are not medicines.

That omission removes the only reliable route to detecting an interaction, since prescribing systems can only check what has been recorded.

The appropriate action for anyone taking prescribed medication is to list every supplement to a pharmacist or clinician, particularly before surgery or a change in prescription.
