Physical activity guidance historically counted the minutes a person spent exercising. Research over the last two decades has established that the hours spent sitting carry their own effects, which are not cancelled by a session at the gym.

The two behaviours are not opposites

Someone can meet the recommended activity target and still be sedentary for most of their waking hours. An hour of exercise leaves fifteen or more hours unaccounted for.

Studies that measure both separately consistently find that sitting time predicts outcomes even after adjusting for how much a person exercises.

The two are therefore treated as distinct exposures rather than as ends of a single scale, which is why current guidance addresses each on its own.

Large muscles switch off during sitting

The postural and leg muscles are the largest in the body and account for a substantial part of resting energy use when they are active.

Sitting with support removes almost all of that demand. Electrical activity in the leg muscles falls close to nothing, which is not the case even during slow standing or walking.

Because these muscles are the main destination for glucose and fat taken from the blood after a meal, switching them off changes what happens to that meal.

Enzyme activity falls within hours

An enzyme in the capillaries of muscle tissue is responsible for pulling fat out of circulating blood. Its activity drops markedly during prolonged inactivity.

The decline begins within hours rather than days, which means the effect is a feature of the day itself rather than of long-term deconditioning.

Standing up and moving restores activity relatively quickly. The response is sensitive to interruption, which is the basis for advice about breaking up long sitting periods.

Breaks matter more than total posture

Studies that compare the same total sitting time distributed differently find that frequent short interruptions produce better glucose handling than one long block.

The interruptions do not need to be strenuous. Standing and walking briefly is enough to reload the muscles and restart the processes that sitting suppressed.

This is why the practical recommendation is about frequency of movement rather than intensity, and why a standing desk alone produces smaller effects than expected.

Why measurement changed the picture

Early studies relied on people estimating their own sitting time, which is unreliable. Wearable accelerometers made it possible to distinguish sitting from standing directly.

That change revealed how much of the day is sedentary in populations that consider themselves active, and how poorly self-report tracked the objective measure.

It also allowed sedentary behaviour to be separated from low fitness, which had previously made the two impossible to distinguish in the data.
